The following list details the necessary data that will facilitate and expedite your patient’s first visit and allow us to give complete instructions for his care:

Diagnosis
Medical Notes, Surgery Practice, Hospital Archive
Copy of all available blood tests, preferably of the last 3-6 months
Copy of all Imaging Reports
Copy of all Pathology reports
